Sodium dependent glucose transporter (SGLT) 1 / 2 - elucidating inhibitor SAR and selectivity using homology modelling and 3D QSAR studies

چکیده
Inhibiting sodium-dependent glucose transporters (SGLTs) has been proposed as a new therapy for the treatment of diabetes [1]. SGLT2 as the most prominent member of this family is mainly expressed in the kidney and responsible for the reabsorption of the vast majority of the filtered glucose. This key role in the blood glucose homeostasis makes SGLT2 a promising target which has been clearly underlined by the results of preclinical and clinical studies. Therapeutic goals of SGLT2 inhibition are reduced plasma glucose levels and weight loss. In conjunction with the therapeutic benefits fewer side effects are expected than observed with other known diabetes drugs. Potential side effects in case of SGLT2 inhibition are expected to be mediated by a lack of selectivity towards SGLT1 that is mainly expressed in the intestine and responsible for glucoseand galactose absorption from food sources. Therefore, inhibition of SGLT1 leads to glucoseand galactose malabsorption, dehydration, and diarrhoea. Consequently, any drug discovery project aiming for promising SGLT2 inhibitors has to take into account significant selectivity towards SGLT1. In order to describe the selectivity on a structural basis we extensively used molecular modelling techniques since no x-ray structural data is available for neither SGLT1 nor SGLT2. For both transporters homology models were generated using the published x-ray structure from vSGLT (PDB-code 3DH4) [2]. Structure based alignments of published SGLT2 inhibitors including selectivity data for SGLT1 were followed by 3D-QSAR studies utilizing CoMFA fields. Given a detailed analysis of the inhibitor SAR for both transporters the two structural models of SGLT1 and SGLT2 were compared leading to the identification of relevant differences and selectivity hot spots. In the future, the presented models could serve as a basis for the identification of new potent SGLT2 inhibitors that are selective towards SGLT1, too.

A functional role for sodium-dependent glucose transport across the blood-brain barrier during oxygen glucose deprivation.
In the current study, we determined the functional significance of sodium-dependent/-independent glucose transporters at the neurovasculature during oxygen glucose deprivation (OGD). Confluent brain endothelial cells cocultured with astrocytes were exposed to varying degrees of in vitro stroke conditions. Glucose transporter (GLUT) 1 and sodium glucose cotransporter (SGLT) activity were investi...

INTRODUCTION Although a large recent trial had shown improved cardiovascular outcomes of diabetic patients on sodium glucose co-transporter-2 (SGLT-2) inhibitors, the influence of gender differences on such outcomes is not known. Thus, we aimed to assess the impact of gender on such outcomes in the patients with type 2 diabetes mellitus receiving SGLT-2 inhibitors. METHODS A search of electro...
متن کاملSodium-Glucose Co-Transporter 2 Inhibitors could Improve the Bioavailability of Vitamin C at the Kidney in Diabetes Treatment
It is known that plasma (serum) vitamin C level is lowered in diabetic patients by some mechanisms including renal loss [1]. Vitamin C and glucose filtered from the glomerulus are presumed to be reabsorbed at the renal proximal tubule by sodium-vitamin C co-transporter (SVCT) 1 and sodium-glucose co-transporter (SGLT) 2, respectively [2-4]. In the enterocyte that expresses SVCT 1 and SGLT 1, it...
متن کاملSodium-Glucose Cotransporter 2 Inhibitors: Possible Anti-Atherosclerotic Effects Beyond Glucose Lowering
The new drug for type 2 diabetes, the sodium-glucose cotransporter 2 (SGLT-2) inhibitor, is reversible inhibitor of SGLT-2, leading to reduction of renal glucose reabsorption and decrease of plasma glucose, in an insulin-independent manner. In addition to glucose control, the management of coronary risk factors is very important for patients with diabetes. Here we reviewed published articles ab...
متن کاملSodium glucose co-transporter-2 inhibitor: Patient safety and clinical importance
The article by Raut and Maheshwari[1] is indeed interesting. However, the suggestion made by the authors on the usefulness of empagliflozin in diabetic perioperative patients needs contemplation from the point of patient safety and risk factors, as it causes euglycaemic diabetic ketoacidosis. Moreover, in May 2015, the Food and Drug Administration issued a warning of ketoacidosis with the use o...
